That trick is typically done by taking a coin and performing a sleight of hand concealement.

There are various concealements that can be done with a coin (classic, thumb, finger palming to name a few).

You basicly make a coin seem to vanish (in reality its concealed in your hand). Then you reach forth with the hand the coin is concealed in and bring it out from behind someone's ear.

The whole time the coin was in your hand at the last second you unconceal at someones ear.
